MENTAL HEALTH THERAPIST

We are looking for a licensed Mental Health Therapists (LPC, LCSW, LMFT or Licensed Psychologist) for PT/FT contracted positions for in-office, home-based, and Telehealth appointments. Preferred candidates will present expertise in varying modalities to provide top-quality care to our clientele. The successful candidate will meet or exceed the following criteria:

  • Current Pennsylvania license as an LPC, LCSW, LMFT or psychologist.

  • Credentialed with health insurance provider panels in Pennsylvania (preferred but not required)

REGISTERED NURSE

We are looking for an experienced home health RN with superior clinical skills to join our Advanced Home Health team. 

 

Position: Registered Nurses/ RN (Home Health)

Status: Full-Time

Position Description: The RN will coordinate the home care of chronically ill patients by utilizing a disease management process, the OASIS assessment, plan of care, and established protocols. Will also educate the patient/caregiver in the management of their illness and promote the home health support systems to decrease re-hospitalization and utilization of hospital emergency services.

 

Qualifications / Requirements:

EDUCATION: Licensed RN or BSN with current PA state license.

EXPERIENCE: Home Health experience preferred.

Ability to work independently and within a multidisciplinary team. Valid driver’s license necessary.

PSYCHIATRIC NURSE

We are looking for an experienced home health RN with a psych background and superior clinical skills to join our Advanced Home Health team.

CERTIFIED NURSING ASSISTANT

We are looking for a Certified Nursing Assistant to join our Advanced Home Health team!

 

Position: CNA (Home Health)

Status: Full-Time and Part-Time

Position Description: The Certified Nursing Assistant (CNA) is a non-professional employee, trained to provide personal care and related services in the home for the sick or disabled when no family member can assume this responsibility. She/he functions under the direction, instruction, and supervision of the staff nurse or appropriate professional staff member.  

 

Qualifications / Requirements:

Retain a Nursing Assistant certification/or the aid will have 16 hours of training within the office of Advanced Personal Care, LLC, and 75 hours of “hands-on” training. Training will be documented and those logs will be maintained in the employee’s permanent record. The HHA will have training in principles of nutrition & meal preparations, the aging process, emotional problems of illness, procedures of maintaining a clean, healthful and pleasant environment, reporting changes in patient condition, and being part of a comprehensive health team.  Ability to work independently and within a multidisciplinary team. Valid driver’s license necessary.

HOME HEALTH AIDE

We are looking for a Home Health Aide to join our Advanced Home Health team!

 

Position: Home Health Aide

Status: Full-Time and Part-Time

Position Description: The Home Aide is a non-professional employee, trained to provide personal care and related services in the home for the sick or disabled when no family member can assume this responsibility. She/he functions under the direction, instruction, and supervision of the Account Manager.

 

Some functions include  

  • Helping the patient to maintain good personal hygiene with a sponge, tub, or bed bath. 

  • Assist in maintaining a healthful, safe environment 

  • Plan and prepare nutritious meals. Markets when instructed to do so by the Account Manager 

  • Assist the patient with ambulation

  • Ability to work independently and within a multidisciplinary team. Valid driver’s license necessary.
