Palliative Care
Palliative care within home health offers an added layer of support for patients whose health is changing while they continue curative or restorative treatments. It serves as a bridge toward comfort-focused care, helping patients manage symptoms, understand their options, and recognize early changes in their condition.

Care remains centered on the patient’s goals, with the team collaborating closely with the patient and family to adjust the plan of care as needs evolve. A hospice-trained nurse partners with the home health team to provide additional education and clinical insight, strengthening the care plan and, when appropriate, helping guide a smooth transition to hospice services.
